Terms, Details and Considerations:

  1. Space is Limited for this even: If you are confident that you would like to attend, please register for this course soon as attendance is limited. (Based on certifying agency guidelines, a maximum of (12) students are allowed to attend any one CPR certification class.)

  2. Refunds: Please submit requests for refunds (by e-mail to: info@DrMills.com) no later than 24 hours prior to the start day and time of this course for a full refund (for this class, no later than 10:00AM on Friday, 8/4/23). If you learn that you are unable to attend after 10:00AM on Friday 8/4/23, or fail to show up altogether, we will gladly apply 100% your registration fee towards a future class.

  3. CPR Skills: By the nature of the CPR instruction, certification process and CPR card issuance, all students who are to receive a CPR card must be able to successfully demonstrate competency in the administration of basic CPR skills such as rescue breaths, chest compressions and basic use of an AED (Automated External Defibrilator) device to receive a CPR card.

(4)-Step REGISTRATION PROCESS:

  1. SUBMIT FORM: Complete and “Submit” the registration form below

  2. WE RECEIVE YOUR FORM AND REQUEST PAYMENT: (Upon receiving your registration request form, we will check current enrollment numbers to make sure that seats in this class are still available). As long as spaces for this class are available (must be 11 or fewer students enrolled at the time that we receive your request/class can accommodate a maximum of 12 total students) we will send (via e-mail) a PayPal payment “request” in the amount of $50 which covers this class, (links to all necessary self-guided academic materials only if “Blended Course” is chosen) and your valid 2-year CPR Card.

  3. SUBMIT YOUR PAYMENT: Simply respond to PayPal payment request. (PayPal and all major credit and debit cards accepted)

  4. CONFIRMATION OF COURSE ENROLLMENT: Once your payment is RECEIVED you are officially registered for this class, and you will be notified of such via e-mail confirmation of official registration acceptance.
